Currently unexplained, but it appears cities need to be a certain point before they turn into barb cities for inactivity.

There a lot of starter cities that never turn into barb cities.

Ok i know what the barb cities are but what about those sort of ghost towns they say they are a player city but they dont have a rank and are basically just like a barb city

Yes buildings grow randomly but troops are not trained
