It's the basic defense value of the barb city. It'll make more sense if you don't use the "civilian defense" explanation. You're right that even figuative defenders, could not cause you to lose the same percentage of troops in a larger wave.
I prefer to think of natural causes of death. Stagnant water, diseased rodents, exposure to the elements, angry deity, etc.
